CE Evaluations Reminders: 

In order to receive credit for sessions, you must complete an online evaluation for each session. You will be provided information on how to access them via email after checking into the Convention. Evaluations will be due Friday July 10th 2015.  


Continuing Education Credit Information:

The 2015 Arizona Pharmacy Association Annual Convention continuing education activities are eligible for ACPE credit; please refer to the final CPE announcement for specific details. This meeting plans to offer 26 continuing pharmacy education activities, including law, patient safety and immunization updates. ACPE credit for these programs has been applied for and meeting registrants may be eligible to obtain up to a maximum of 16 contact hours (1.6 CEUs) over the three-day conference.


CPE activities at the 2015 AzPA Annual Convention will be knowledge-based and application-based learning activities for both pharmacists and pharmacy technicians. 

 

Refund Policy: 

Refund and/or cancellation requests must be made in writing to the Arizona Pharmacy Association office. A $50 service charge will apply to all cancellations before June 11th 2015.  Substitutions are welcome. Refund checks will be mailed two weeks after the conclusion of the annual convention. After June 11th 2015, a $150 charge will apply to all cancellation requests.  AzPA reserves the right to cancel any activity because of in sufficient pre-registration as determined by the Executive Director or CE Administrator. In this case, all individuals pre-registered will be notified and issued a full refund.  


 The Arizona Pharmacy Association is approved by the Accreditation Council on Pharmacy Education as a provider of continuing pharmacy education.
