With expanded use of anticoagulant agents, the number of patients receiving these drugs has increased dramatically. Safe and effective anticoagulation must include a number of key components to avoid complications. These include careful patient assessment, an understanding of the clotting cascade and mechanisms of action of anticoagulant therapies, a detailed focus on factors which influence therapy and knowledge of current guidelines. This certificate program is a comprehensive program designed to provide pharmacists with the basic knowledge and skills necessary to care for patients taking anticoagulation therapies. By completing this course, pharmacists can earn 22 contact hours of continuing pharmacy education credit while becoming certified in anticoagulation management.



(14 HOURS HOME-STUDY/ 1.4 CEUs) application-based learning activity 

The didactic home study will consist of:

1) Reading selected monographs relating to the 2012 ACCP Chest Evidence-Based Guidelines,

2) View the 2 video links below,

3) Completing a study guide with assessment questions, and

4) Complete an on-line home-study exam. Note: A score of at least 70% or better is needed in order to qualify as passed.

Upon registration, you will receive a link to the home study program book, which includes the study guide assessment, and 2 videos. In addition you will be required to take a home-study exam. The home-study exam must be completed prior to the live session -June 9th, 2016. You will need to bring your study guide with you to the live session. As you check in to the live session we will confirm your completion of the study guide assessment. Pharmacists will receive CPE credits of a total of 14.0 contact hours or 1.4 CEUs for the home study portion of this program.


  • Differentiate the pharmacology and pharmacokinetics of UFH, LMWH, warfarin, direct thrombin inhibitors, and factor XA inhibitors
  • Define the current recommendations for indication-specific intensity and duration of anticoagulation treatment
  • Recognize strategies used to manage hemorrhagic complications and HIT associated with anticoagulant therapy
  • Identify appropriate methods to manage antithrombotic therapy during the perioperative time period


(8 HOURS LIVE CREDITS / 0.8 CEUs) application-based learning activity


The live program involves 8.0 contact hours (0.80 CEUs) of both lecture and simulation training. Participants will focus on the pathophysiology of thrombosis, receive an overview of ACCP Guidelines, discuss indications for anticoagulation therapy, warfarin dosing and monitoring, management of the over-anticoagulated patient and patients requiring procedure management, patient education, and patient interviewing and assessment. Participants will also learn about point-of care monitoring devices, and practice assessing patients through the use of patient cases. At the conclusion of the live program each participant must complete the on-line final exam with a score of 70% or above and a program evaluation in order to receive a certificate of completion. Pharmacists will receive a total of 8.0 contact hours or 0.8 CEUs of continuing pharmacy education for completion of this component. 


  • Review pathophysiology of thrombosis
  • Describe the difference in mechanism of action between the new and old antithrombotic therapies
  • Describe the indications for anticoagulation therapy
  • Review current treatment guidelines
  • Evaluate the initiation and dosing of warfarin
  • Determine factors influencing dosing of warfarin and the appropriate management of out-of-range INRs
  • Develop assessment tools which account for variables impacting warfarin intensity and dosing
  • Compare and contrast available point of care devices for anticoagulation therapies and demonstrate correct patient care procedures for testing
  • Evaluate ‘bridging” therapies for anticoagulation management
  • Practice assessing patients on anticoagulation therapies through the use of patient cases

Anticoagulation Certificate: 
In order to receive your certificate of completion, you must successfully complete both the home-study and the live portion of the Anticoagulation Certificate Program.

Continuing Pharmacy Education (CPE) Credit and Activity Requirements:

 The Anticoagulation Certificate Program has been approved for 22.0 contact hours or (2.20 CEUs)

  • Learning Level: 2 — Experience with the topic is recommended, but advanced expertise or specialty practice in the subject area is not necessary. This activity will focus on expanding the learner’s knowledge and application of new and/or more complex information.
  •  Target Audience:  Pharmacists in all practice settings 
  • Successful completion of the home-study component involves passing the home-study exam with a grade of 70% or higher and will result in 14.0 contact hours of CPE credits (1.40 CEUs). 
  • Successful completion of the live seminar component involves attending the full live seminar, passing the final exam with a grade of 70% or higher and completing a final evaluation. Successful completion of this component will result in 8.0 contact hours of CPE credit (0.80 CEUs). 

Statements of Credit will be available online within 60 days on participant’s CPE Monitor profile at The Certificate of Achievement will be submitted electronically to all participants 30 days upon successful completion of both the home-study and live seminar.


Activity Completion Requirements:

A Certificate of Achievement is awarded to participants who successfully complete all activity requirements, which include the self assessments, the home-study activity, the home-study exam, the live training seminar, and the final exam and evaluation. The self-study materials must be completed prior to the live session on Thursday, June 9th, 2016. The final exam and evaluation must be completed no later than June 26th, 2016. We will upload all credits 30 days from the live session date. It is the participants responsibility to ensure your credits are uploaded within 60 days from the live session date. After this time you will no longer be able to claim your CPE credit.


Refund and/or cancellation requests must be made in writing to the Arizona Pharmacy Association office. A cancellation request made at least two weeks prior to the scheduled program will require $100.00 fee (cancellation fee plus the cost of printed materials). A cancellation request made 10 business days or less prior to the schedule program will require a $150 fee (cancellation plus printed materials, plus cost of food order). You must submit a written request to cancel by email, fax or standard mail. No refunds will be issued for cancellation requests received after the posted start date / time of the scheduled program. AzPA reserves the right to cancel any activity due to insufficient pre-registration and as determined by the Executive Director or CE Administrator. In this case, all individuals pre-registered are notified and issued a full refund. Registration fees can not be re-allocated to future offerings of this certification program due to the multiple steps involved with processing CE credits.
